Voluntary Life
100% Employee Paid Benefit:

- Available in increments of $25,000
- Up to $250,000
- Guarantee issue = $50,000
Spouse:
- Employee must elect voluntary life insurance
- Cannot exceed 50% of employee’s amount
- Available in increments of $5,000
- Up to $125,000 if you apply within 31 days of eligibility
- Guarantee issue = 50% of employee’s amount up to $25,000
Dependent:
- Available in increments of $2,500 up to $10,000
- No evidence of good health required

Disability Insurance is “Income Protection Insurance”
Income Protection Insurance pays a monthly benefit for a disability resulting from a covered injury or sickness benefit:
- 60% of pre-disability earnings to a maximum of $1,000
- Elimination period is 90 days
- Definition of disability: two years
- 3/12 Pre-existing condition

We recognize that PTO is important to maintain a positive and productive work environment by allowing Hill Yorkers greater flexibility in scheduling time off for vacations, illness, appointments, family emergencies, and other such needs.
PTO is accrued or earned based on position classification, length of service and anniversary date.
